Learning from the things that go wrong . . .
Last July 3 to 6, 2007, I was privilege to attend Bob Nelms training at Jakarta, Indonesia on the Latent Cause Experience, & my thoughts regarding Root Cause Analysis had been truly enhanced and sharpened. My purpose of joining this training is to become an Affiliate and guide industries interested in learning from the things that go wrong.
The Latent Cause Analysis experience is trying to understand why things go wrong so that we can learn from them. Many people think failure in a negative fashion, people do not want to hear our failures, only success. Yet we do not see that for every single failure, there is always a lesson to be learned, where in fact failure is our greatest teacher and we can only be successful when we learn from our failures. |

Every single failure we encountered are caused by people and what do we do about these failures, we fix them and when they recur, we fix them again until we become good at fixing them. And for those who cause the failure, we punish them, we wanted someone to be blame and take the fall and only one can say that it is part of human nature. But what we don’t realize is that for us to understand, the way people think about failures is that we must be willing to see ourselves as part of the problem. If people cause problems, then they are also worth of providing solution to problems. |
Latent Cause Analysis is all about asking ourselves the following :
- What is it about the way we are that contributes to our problems ? - What are we going to do about it ?
Most industries have their own form and definition of Root Cause Analysis and many people from industries have many interpretations as to what the real Root Cause is, but 1 problem that adds to their confusion Is not knowing where to stop or how deep their RCA efforts should be, while others are merely satisfied on finding who really caused the failure.
